Instructions for New Patients

PLEASE NOTE: At this time, Dr. Bibeault is accepting a limited number of new patients. Depending on your needs, you may be placed in the schedule, invited to be on the wait list, or given alternative referrals for care. Please contact the doctor’s schedulers to obtain the most current information.

Confidentiality
Dr. Bibeault keeps your status as her patient, as well as all information that you discuss, confidential. However, some rare situations legally require her to break confidentiality. These include cases where Dr. Bibeault suspects imminent self-injury by the patient, imminent harm to another individual, cases of elder or child abuse, or when she has received a judge-issued court order to relinquish treatment records.

To secure your appointment time, please register as a new patient by submitting the registration form below. You may mail or fax the completed form to Jennifer at:

2. Print the following 2 forms (if you don't have Adobe Reader, you can get it here), complete them, and bring them with you to your appointment:

Consent to Release Information
Health History Questionnaire

3. Also bring with you to the appointment:
  a. Medications you're currently taking
  b. A form of payment (cash or check)
  c. Any medical records that might be useful in your evaluation
